Wendy Rotolo RN, MSN, MHA, OCN

Our Case Management team at Leonard J. Chabert Medical Center provides service for patients in partnership with the physicians, nurses and other supportive interdisciplinary teams. Registered Nurses and Social Service counselors work together toward the ultimate goal of moving patients safely and expediently to the next level of care along the health care continuum. Social Service personnel are also available to care for psychosocial needs as well as discharge planning support for our patients.

Please see our monthly newsletter, posted on this site which provides valuable information and education for providers and nurses to help keep up with the dynamic changes in regulatory rules and compliance topics related to case management.

I applaud our team for their dedication and effectiveness. As with all Chabert departments, our team views their role as a family, here to put our patients first.
