The Diabetic Care Clinic at Lallie Kemp Medical Center works with patients referred by their primary care provider to help manage difficult-to-control Type I and Type II Diabetes Mellitus.  Patients referred to our clinic have a Hemoglobin A1C of 9 or greater. Our Diabetic Care team works together using a multi-disciplinary approach to create a customized treatment plan for each patient.  Patients work closely with a nurse, registered dietician and nurse practitioner at every visit.
