Leonard J. Chabert Medical Center, better known to local residents as South Louisiana Medical Center, was officially opened in 1978. The medical center was built to serve the health care needs of the populations of Terrebonne, Lafourche, Assumption, western St. James and eastern St. Mary parishes regardless of financial determination. The hospital is licensed for 147 beds, and is a JCAHO accredited institution.

Additionally, the hospital was established as a teaching facility to provide clinical training for physicians and Allied Health Services personnel. South Louisiana Medical Associates (S.L.M.A.) was formed in 1978 as an extended corporate arm of the Alton Ochsner Medical Institution to train residents. LSU and Tulane pre-med students also rotate through the hospital.

In 1997, Louisiana State University Health Sciences Center/Health Care Services Division was authorized to assume the administrative functions of the hospital. LSUHSC/Leonard J. Chabert Medical Center and South Louisiana Medical Associates combine to provide Southeast Louisiana with the finest educational and medical services.

L. J. Chabert Medical Center is the flagship of Disease Management within the HCSD Disease Management Programs in Diabetes, Asthma, HIV-AIDS and Cancer Screening. It is also home to an outstanding Diabetes Education and Wellness program.

Ms. Rhonda Green currently serves as the Chief Executive Officer at Leonard J. Chabert Medical Center. Ms. Green is a Registered Nurse boasting over seventeen years of clinical experience and management in the health care setting. Her last 31/2 years were spent functioning as the Chief Nursing Officer at the state run facility in Houma. Prior to assuming her role at Chabert, Ms. Green served in leadership capacities at the sister institutions of Hotel Dieu and MCLNO as well as other private hospitals in New Orleans. Her educational background includes a BSN in nursing, a Masters Degree in Business and a Master's Degree in Healthcare Management.
