LSU HCSD IRS Form 1095-C Notice
As part of the Affordable Care Act (ACA), LSUHSC-HCSD is required to provide information about your health coverage for the 2025 tax year. However, LSU HCSD is no longer required to issue paper copies of Form 1095-C through the mail.

How to Get Your Form (If Needed):
While you do not need this form to file your tax return, you may request a copy for your records. You can obtain your 1095-C by any of the following methods:

LSU HCSD will provide your Form 1095-C within 30 days of your request.
